SilverSearch™ for Patient Transition to Post-Acute Care

How many patients does your hospital discharge to post-acute care annually?

In the Report to Congress: Medicare Payment Policy - March 2015, Medicare reported that "42 percent of patients discharged from an acute care hospital went on to receive post-acute care."

Medicare paid for 9.6 million post-acute care encounters to more than 29,000 post-acute care providers in one year.

Hospital Discharge Teams Use SilverSearch to Help Patients Choose Post-Acute Care Providers

SilverSearch™ is a searchable tablet application that acute-care hospital case managers and discharge teams use to help patients choose their post-acute care (PAC) provider. The tablet provides patient-facing CMS ratings and information about PAC providers throughout the U.S. With this comprehensive and robust application, patients can easily filter searches based on the specialized care needed (including long-term acute care, inpatient rehabilitation facility, skilled nursing facility, and home health agency) the preferred location and suitable radius, and the accepted insurance plan(s).

SilverSearch Supports Compliance

IMPROVING MEDICARE POST-ACUTE CARE TRANSFORMATION ACT OF 2014 (IMPACT ACT) defines initiatives and measures to improve the quality of post-acute care. The Centers for Medicare & Medicaid Services (CMS) has proposed revisions to the discharge planning requirements of the IMPACT Act that hospitals (including long-term care hospitals and inpatient rehabilitation facilities, critical access hospitals, and home health agencies) must meet in order to participate in the Medicare and Medicaid programs. SilverVue tablets help hospital case managers comply with CMS initiatives and the proposed patient discharge IMPACT Act rule.

The Medicare Outpatient Observation Notice (MOON) "requires both written and oral notification to individuals receiving observation services as outpatients for more than 24 hours explaining the status of the individual as an outpatient, not an inpatient, and the implications of such status." SilverSign™ e-signature on the SilverVue tablet simplifies patient signature of the MOON form and other documents. Completed documentation is posted to the EHR.
