Hospital Staff Use SilverSearch™ at Bedside, Workstations, and Remotely to Help Patients Achieve Better Outcomes

SANDY, UTAH – April 20, 2017 – SilverVue®, a provider of hospital patient transitional care technologies, is showcasing SilverSearch™ at the ACMA 2017 Annual Case Management Conference located at the Gaylord National Resort & Convention Center in National Harbor, Maryland on April 20-22, 2017.
SilverSearch, designed by case managers for case managers, supports hospital discharge workflows assisting patients in post-acute care selection. SilverSearch is presented at bedside on a tablet or is accessed through a newly released online portal.
Patients easily compare post-acute care (PAC) provider information on a provided SilverVue tablet allowing for an informed and compliant choice in their next step of care. Post-acute information is sorted and filtered to meet the patient’s specific needs. Provider information is emailed to family or other care providers to assist patients in their care decision. Printed customized lists can also be produced from the SilverSearch online portal.
Case managers use SilverSign™, a SilverSearch add-on module, to electronically collect patient’s signature at bedside. Any forms can be added to the tablet (e.g., “Patient Preference” form, “MOON” form) after which once e-signed, post directly to the EHR.
Hospitals leverage SilverVue to filter providers who have been qualified by the Centers of Medicare and Medicaid Services (CMS) participating in The Comprehensive Care for Joint Replacement (CJR) program. Custom networks, preferred, and affiliated providers can also be filtered to more succinctly support hospital initiatives. Patient-facing CMS measures for post-acute care providers support compliance with the proposed CMS IMPACT Act rule.
Hospital leadership can utilize reported search data to create and better support management initiatives. Digital resources frequently used by discharge staff are managed and added to the SilverVue tablets for immediate current access of information.
