Who Uses SilverSearch?
Hospital Case Managers
Hospital case managers provide patients with the searchable SilverVue digital directory so that patients are engaged in selecting the provider that meets their preferences as they navigate the next step in their care. SilverVue technology eliminates cumbersome paper lists and brochures so that patients can make better choices and hospitals can easily document patient preferences.
SilverSearch helps hospitals comply with the CMS proposed patient discharge IMPACT Act new rule and other patient choice initiatives.

Patients can easily filter searches on the SilverVue tablet based on the specialized care needed (including, long-term acute care (LTAC), inpatient rehabilitation facility (IRF), skilled nursing facility (SNF), and home health agency (HHA)). The patient then selects a radius by zip code anywhere in the U.S. and the accepted insurance plan(s).
SilverSearch helps to improve the patient’s experience and engagement. Patients who participate in their care planning are more confident and less anxious about their transitional care.
Post-Acute Care (PAC) Providers
The SilverSearch tablet contains a compiled database listing of all U.S. PAC providers. There is no cost to be listed and it is automatic. The listings display the agency’s address, phone number, CMS ratings, distance, and other information.
SilverVue provides free access and login information to PACs so that they can update important information, such as, the accepted insurance plans, and manage their profile. These updates upload automatically to the SilverVue listings so that patients anywhere in the U.S. can see real-time information and supports hospital collaboration. PAC providers are encouraged to contact SilverVue to access and manage information about their agency with no cost or obligation.
